Yes, it does require that you have all your project requirements very detailed and labor division correctly divided up. You also need to be very detailed on project management. Its not an easy process but it can be done.

Yes. My company has multiple designers and we all talk threw email and chat clients. I think if you know the other programmers well enough you will not have a problem.

I participated in a couple projects in college like that with 2 or 3 people and they went pretty smooth although they were very basic projects.
Recently I participated in a web community lead project to write a forum software. Everything was very well planned and detailed but it took less than 2 weeks for things to decend into chaos and the project folded.
It all depends on the people you are working with, their skills and maturity.
